2014 Medical & Activity Release Forms

This is the latest medical form for 2014. Everyone needs to complete parts A, B1, B2 (2 - B pages).

Those going to Summer Camp or High Adventure also MUST have part-C completed by a Doctor.

Also provided is the official Boy Scouts of America Activity Consent & Adult Approval Form (680-673 rev2012).
Troop 2020 is adopting this form for all Troop activities outside of troop and patrol meetings (Service Projects, Camping, Summer Camp, Movies, etc.) Any participants under 21 will be required to have a signed form turned in at the time of leaving for the activity.
